The initial history, examination and subsequent visits are conducted in their entirety by a specialist who will be primarily responsible for your treatment. The same specialist orders and assesses the necessary tests and coordinates the evaluation by other specialists when appropriate.

Your Initial Telephone Call
Please call us anytime at (626) 445-8481. If you call during office hours someone will tend to your call and answer any questions you may have about our physicians and services. If you call when the office is closed, messages are taken by our 24-hour answering service. Those messages are responded to promptly on the next business day.
We will not make you wait weeks or months to get help. We will try to schedule you right away.
What to Expect During Your Initial Visit
Plan to spend two hours in the office. You will fill out a thorough medical questionnaire. It is very important for the neurologist to have comprehensive information about your health. Ask our staff if you need any assistance — we are happy to help you with any questions you may have.
We pride ourselves in listening to our patients carefully and are responsive and receptive to questions or suggestions that you may have. We keep a friendly and caring attitude and have a keen awareness that we are dealing with a human being–not just a disease or a medical condition.
During the course of your treatment, you may occasionally need a telephone consultation with the doctor. You may call any time (626) 445-8481 and leave a message with our staff for your specialist. Messages are answered within 24 hours except on weekends and holidays. Please note when the office is closed, messages are taken by our 24-hour answering service. Those messages are responded to promptly on the next business day.
During your visit, the Doctor will insure that you have enough medication until your next office visit. However, under unusual circumstances, you may anticipate running out before the next appointment. In that case, call your pharmacist and ask him/her to fax/e the prescription refill request to (626) 574-9669. The prescription will be approved and faxed/e back to the pharmacy within 24 hours with the exception of weekends and holidays. Please note a refill request can only be approved for patients who have been seen recently. Finally, please remember to allow sufficient time to renew your prescription so you don’t run out of your current prescription.
Narcotic medications cannot be refilled on the telephone.
Narcotic medications or their prescriptions cannot be replaced if lost for any reason.
Prescription of a new medication that our Doctor has never prescribed usually requires an office visit.
Canceling Your Appointment
If you are unable to keep your appointment, we appreciate notification of at least 24 hours before your scheduled appointment. Our office personnel will be happy to reschedule your appointment. Please call (626) 445-8481.
As a courtesy we will attempt to remind you of your scheduled appointment time, but it is the responsibility of the patient to remember to keep the appointment.
Emergencies are serious problems that cannot be treated over the telephone. If this is an extreme emergency, call 911 immediately.
If you have a non-life threatening emergency, call (626) 445-8481 at any time. If the emergency occurs during office hours and it is not life threatening you will be told to come to the office unless the emergency is of such nature that it cannot be handled in the office. In that case, you will be instructed to go to the Emergency Room of your local hospital. If the emergency occurs during any other time (the office is not open), you will be told to go to the Emergency Room of your nearest hospital. The Emergency Room physician will contact our doctor.
We accept most major insurance. Please check with your insurance carrier to determine your benefits. Our office can provide you with other specific information. If you have a question about your specific plan, please call (626) 445-8481. We will be glad to look into your insurance plan specifically.
Below are some of the most common plans that we accept:
- Blue Cross/Blue Shield
- United Healthcare
- Provider to Medicare
- Healthcare Partners HMO
- Worker’s Compensation Carriers
- Any money due for your services (including copays, deductibles and self-pay patients) will be collected at the time of your appointment.
- We accept Visa and Mastercard.
- Any charges that have been cleared by your insurance plan (i.e. deductibles, patient percentage, services not covered by your plan) are due upon billing and may be paid at the office.
- Patients with balances older than ninety (90) days will be referred to the business office staff or the operations manager.
Request a Copy of Your Medical Records Form
California law prohibits the disclosure of records that contain a patient’s medical information by any person or entity without first obtaining a valid authorization for release of the information. The law also gives patients the right to cancel or revoke their authorization at any time. Please submit a completed authorization form to our office in person, by fax or mail.
